A Scope of Appointment is required before a personalized consultation about Medicare Advantage or Part D plan options, including by phone, video, or in person. General educational questions do not require one. The intake form permits a response and does not authorize enrollment.
Before any Marketplace application or update is submitted, the applicant must separately review the eligibility information and confirm that it is accurate. The intake form and the Marketplace Client Consent Form do not replace that separate confirmation.
